To obtain the Eternal Mangekyo Sharingan, is it truly necessary to transplant a brother's or rather close related blood relatives eyes? What if one has no blood relative left? Must one resort to cloning their own eyes and merging them?
Consider Uchiha Obito. After implanting Hashirama's cells, he could use the Mangekyo power without consequence. Was it simply because Senju Hashirama's cells were that miraculous?
No. It was the power of Yang Release Chakra.
The growth of the Sharingan relies on Dojutsu, and the Dojutsu at it's core is highly concentrated Yin Release Chakra. The Senju clan represents Yang Release. Hashirama's Wood Release was the physical manifestation of that high-concentration Yang Release Chakra, having an affinity for Water and Earth attributes was merely a sufficient condition, not the cause.
There are plenty of ninja in the world with dual Water and Earth natures, yet aside from Hashirama, none have naturally awakened Wood Release. Yamato's Wood Release relies entirely on Hashirama's cells. Because the Yang Release within Yamato's own body pales in comparison to the original, his version of Wood Release is vastly inferior.
At its core, the shinobi world revolve around Yin-Yang Theory.
It was precisely for this reason that Madara, after obtaining a piece of Hashirama's flesh, spent decades trying to Develop the Rinnegan. He was relying on the trace amounts of Yang Release Chakra contained within that single graft.
Because the amount of chakra in a single piece of flesh was so minuscule, it took Madara a lifetime to succeed. If Hashirama himself had been there to pour high-concentration Yang Release Chakra into him on a daily basis, Madara's eyes would likely have evolved much faster.
Therefore, theoretically, one does not strictly need to absorb the Dojutsu of a sibling's Mangekyo. If one possesses a sufficient supply of high-concentration Yang Release Chakra, it can solve the issue of deteriorating vision and drive the eyes to evolve.
The correct method, after awakening the Mangekyo Sharingan, is to acquire a vast amount of high-concentration Yang Release Chakra. This allows Yin and Yang to circulate and harmonize, eventually evolving the eyes into the Eternal Mangekyo Sharingan and, finally, the Rinnegan.
Madara's theory, that a brother's eyes are required for the Eternal Mangekyo Sharingan, wasn't technically wrong, but his method was crude. He achieved it by forcibly stacking high-concentration Yin Release (Dojutsu) upon high-concentration Yin Release (Dojutsu) to force a promotion, to produce sufficient Yang Release to balance out excessive stress Dojutsu put's on the eyes and brain.
Essentially, Madara's method of evolution was a crude path.
Currently, the Yang Release Chakra within Naruto is not far behind that of Senju Hashirama. Theoretically, his power could facilitate the upgrade of Sasuke's eyes. The proof lay in the fact that when Naruto shared his chakra with Sasuke, he did not bleed tears like Itachi when using his Mangekyo power. Instead, like Obito, he used it as if it were no burden at all.
"It's a pity... you don't need this power." Naruto shook the bottle containing Itachi's eyes with a casual rattle. "And I don't need this power either."
'If you don't want it, give it to us!'
Many in the crowd stared at the preserved eyes, their own gazes burning red with envy, screaming internally. They were also cursing Sasuke in their hearts for being a fool. Except for Fugaku, he still blamed himself for how Itachi turned out and had similar opinion as Sasuke on this part.
That was the power of the Mangekyo, Sasuke could directly evolve his Mangekyo into Eternal Mangekyo! Obtaining such power was tantamount to ascending to Super Kage tier in a single step. After all, Senju Hashirama was titled the God of Shinobi, how far behind could the legendary Madara truly be?
If you, Sasuke, don't want them, there are plenty of people who do.
Unfortunately for them, Naruto had no intention of handing them over.
"Give the sinner's eyes to me."
A single figure stepped forward from the ranks of Root. It was Izumi Uchiha.
She slowly removed her mask. Showing her eye patch, covering the eye she had lost to the blindness of Izanagi.
Izumi took the bottle from Naruto and gazed at Itachi's preserved eyes.
"Izumi-nee?" Sasuke stared blankly at her. Although the years had changed her slightly, he recognized her immediately.
"Sasuke... you have suffered these past years." Fugaku also slowly removed his Root mask. "As expected of my son."
"Fa... Father?" Sasuke looked at Fugaku, his mind grinding to a halt.
'Clan Head Fugaku?!'
If few people recognized Izumi, Fugaku's unmasking caused a massive stir among the assembled ninja clans. Then, one by one, the other Root members removed their masks.
"You... You guys...?"
For a moment, Sasuke thought he had fallen into a trap. He suspected he was still trapped within the illusion woven by Itachi, that everything happening now was false.
Sasuke activated his Mangekyo Sharingan, frantically scanning the faces of the clansmen appearing before him. He stared for almost a minute, searching for a flaw in the chakra flow, but found no anomalies.
'They are all real?'
These Root Ninja... were all Uchiha?
Aside from the Hyuga, the other clans were in a state of shock. Weren't they dead? The clan had been exterminated years ago.
'I saw those hundreds of Uchiha buried with my own eyes.'
"The Uchiha did indeed experience a Clan Extermination," Fugaku spoke again, addressing both the stunned crowd and his son. "Most of the Uchiha died in that disaster. However, a small portion of the clan survived."
"I see. It's the hundred or so people who went missing," Shikaku said slowly, piecing it together.
A thousand Uchiha. Nine hundred dead. One hundred bodies never recovered. Because traces of Root Ninja had been found nearby, even though Danzo refused to admit it, the village had classified them as missing rather than confirmed dead. Most people didn't know the details, but Shikaku was privy to some of the internal reports.
"Exactly." Fugaku looked at Sasuke, his expression heavy with apology. "I'm sorry, Sasuke."
Sasuke abruptly turned his head to look at Naruto, his eyes practically screaming the question: 'Why did you keep this from me too?'
"Not my business. Uncle and your fellow clan members wouldn't let me tell you." Naruto shrugged. "They hoped the pressure would help you grow quickly."
"And they were proven right. Your strength has improved rapidly, now you've even awakened the Mangekyo."
Sasuke's eyes instantly turned red. But this time, it wasn't the crimson of the Sharingan, it was the raw redness of his eye sockets filling with tears. 'Why didn't any of you tell me?!'
At that moment, Sasuke felt incredibly aggrieved. He thought he had lost everything, only to find that everything he cherished had returned. He didn't know how to express the storm of emotions swirling inside him. He could only think...
'Happiness came too suddenly.'
"Initially, we should have told you before this." Fugaku scratched his cheek. On a face usually as immutable as an iceberg, a rare look of awkwardness appeared. "But a situation arose recently, and I couldn't get away."
Fugaku certainly wasn't going to tell Sasuke that he and Mikoto had been so preoccupied with this "situation" that they had almost forgotten about him.
'What matter could possibly be more important than me? Your own son!?' Sasuke pursed his lips.
"Sasuke, this is actually good news." Naruto patted Sasuke on the shoulder. "Congratulations."
Sasuke's confusion vanished as his attention was completely captured by Fugaku's next sentence. The feeling of grievance evaporated instantly.
"Mikoto... she... was pregnant with another child."
"The baby was born just a few months ago."
Sasuke's eyes went wide.
Mother was pregnant with another child? Does that mean, I've become a big brother?
"This time, it's a younger sister!"
"What?!"
