Now, Konoha Prison held only one prisoner: Uchiha Sasuke.
With his current strength, he could have easily escaped… if he hadn't been wrecked by Guts before leaving.

Seeing Guts and Shisui appear during the war gave Sasuke a very bad feeling.

When Guts and Shisui finally stepped out, Shisui was holding a transparent cylinder filled with nutrient solution, Inside floated a pair of Sharingan.
"Go," Guts said, wiping the blood from his hands.
"…Yes."
Shisui hesitated for two seconds, then followed.
By dawn, the two arrived at the medical station where Tsunade, Kurenai, and Shizune were working.
"Tsunade! Get the best medical ninja! We need an eye transplant, now!"
Even at this hour, medics were still rushing to treat injured shinobi.
Hearing Guts's voice, Tsunade, Kurenai, and Shizune all hurried out of the tent.
"What's going on?"
Tsunade and Kurenai both looked confused at the sudden arrival.
"I'll explain later. For now, transplant these eyes to him." Guts pointed at Shisui.
Tsunade glanced at Shisui, then at the jar he was holding.
"…Shizune, Kurenai! Prep the operating room. Sakura, fetch every tool for an ocular transplant!"
"Yes!"
The medics scattered to prepare.
Tsunade took the jar from Shisui, stared at the Sharingan for a few seconds, then barked, "Bring him in!"
Guts sat down outside the operating room, waiting silently.
Killing Kabuto would be easy but without disarming Impure World Reincarnation, the war wouldn't end.
For that, he needed a Genjutsu grandmaster.
And Shisui… was the perfect candidate.
Inside, Shisui lay on the table. His old eyes had been removed, leaving only bloodied sockets.
The new Sharingan were carefully treated and placed into his eye sockets.
"Lady Tsunade… whose eyes are these?"
Sakura, wearing gloves and a head cover, couldn't hold back her unease.
"Shh!"
Shizune immediately hushed her. "Sakura, don't distract Lady Tsunade at a critical moment!"
"…Yes. I'm sorry."
Sakura lowered her head, though dread lingered in her chest.
After what felt like hours, Tsunade and Kurenai emerged, sweat on their faces.
Shisui followed, eyes wrapped in bandages, nearly stumbling from disorientation.
"Done?" Guts stood immediately. "Good. Then we're leaving."
"He really should rest first," Shizune protested.
"There's no time," Shisui said firmly. "If we don't stop Impure World Reincarnation, the casualties will only grow."
Guts smirked. "How do the new eyes feel?"
Shisui touched his bandages and hesitated.
"…I can feel an unprecedented Visual Prowess."
"With your skill, you'll master them quickly," Guts said, tone leaving no room for doubt.
"Yes."
"Guts-sensei… whose eyes did he—? They're Sharingan, right?" Sakura finally asked, her voice trembling.
Guts let out a strange laugh.
"You'll know… after the war."
The two men turned and disappeared into the night, leaving the girls frozen.
Back when they buried Uchiha Fugaku and the others, they hadn't gouged out eyes like Danzo.
Options for a Sharingan transplant were… limited.
Sakura's mind trembled with a terrifying possibility.
She shook her head violently, trying to deny it.
Impossible… No. I'm just overthinking…
