Chapter 1 - Medical Ninja System

Konoha Hospital.

Early in the morning, Hirakawa Aoshi arrived at the hospital—not as a patient, but as a full-fledged medical ninja reporting for his first official day of work.

Just yesterday, he completed his internship. Starting today, he was officially recognized as a qualified medical ninja.

"Good morning, Lord Aoshi!"

The moment he stepped through the entrance, several nurses greeted him warmly. Their eyes lit up as they looked at the blue-haired, golden-eyed Aoshi, admiration and respect plain on their faces.

"Good morning!" Aoshi responded with a bright, friendly smile, returning their greeting without the slightest trace of superiority, and continued down the corridor toward his assigned office.

"As expected of Lord Aoshi! He's exactly my type!"

"Keep dreaming. With your civilian-level skill, you think you're a match for someone like him?"

It had been barely a year since the end of the Second Great Ninja War, and Konoha was still struggling to rebuild its depleted ranks of medical ninja. In such times, a young man like Aoshi—talented, dedicated, and undeniably handsome—naturally held in high esteem.

A rare genius. A name already spread across the Konoha hospital.

Sitting at his desk, Aoshi exhaled quietly and focused his mind. In that moment, a translucent panel flickered to life before his eyes—visible to him alone:

There was no doubt—Hirakawa Aoshi was a transmigrator. Eighteen years ago, he had been reborn in Konoha.

The good news? He got a second chance at life.

The bad news? This was the Ninja world—a place where killing came easy, and bloodlines ruled everything. And unless you were born with a legendary surname or a Kekkei Genkai, the road ahead was mercilessly steep.

As a ninja without a bloodline, survival here was harsh. Aoshi was living proof.

At the Ninja Academy, he was never among the elite. His physical skills were average at best, and his combat scores rarely stood out. The only thing that kept him afloat was his unmatched performance in theory; he consistently ranked at the top in written exams.

Then the Second Great Ninja War broke out. At the age of fourteen, he was sent to the front lines. Though he survived the war, his jonin instructor and a teammate died in battle. His team disbanded soon after.

Afterwards, he planned to return to the ninja academy and become a teacher. But fate had other plans. One night, he suddenly awakened the Medical Ninja System and unlocked an extraordinary affinity for medical ninjutsu.

Techniques that took others years to master came naturally to him. Step by step, he advanced beyond the limits of conventional learning.

In just one year, he went from knowing nothing about medicine to becoming a respected medical ninja. It was an incredible pace—especially considering a portion of that year was spent on theory alone.

In truth, he had mastered several medical jutsu, including the B-Rank Delicate Illness Extraction Technique, in only a single week.

In the shinobi world, jutsu are ranked by difficulty: E, D, C, B, A, and S. B-Rank jutsu were considered jonin-level.

And medical jutsu were even harder to learn than standard ninjutsu. The precise chakra control required to mend flesh, stop bleeding, and repair internal damage was something even elite ninjas struggled to master.

Which made Aoshi's progress all the more extraordinary.

Of course, the system didn't stop there. Every time Aoshi successfully treated a patient as the lead Medical Ninja, he was rewarded with an Orb.

Assisting others, relying on clones, or healing himself didn't count. Only when he took full responsibility would the system recognize his efforts.

And starting today, he was finally qualified to treat patients independently. No more shadowing senior medics. No more assisting from the sidelines.

"Lord Aoshi! We have a patient who needs immediate attention!" A nurse rushed over and knocked urgently.

"Understood! Prep the operating room immediately!"

Suppressing his other thoughts, Aoshi hurried to the surgical room.

But the moment he stepped into the room and laid eyes on the patient, he was stunned. It was someone he knew.

"…Mikoto!"

Uchiha Mikoto—his classmate from the Ninja Academy and a former teammate from the same genin squad after graduation.

It had been a year since their team disbanded. In that time, Mikoto had advanced quickly, earning her promotion to jonin and now served as a Team leader for a new generation of genin.

But at this moment, she lay unconscious on the hospital bed, covered in blood. Her abdomen had been crudely treated, but the wound still bled profusely, soaking through the white cloth and dyeing it crimson.

Due to the severe blood loss, her face had turned deathly pale. If not for her faint breathing, one might mistake her for a corpse.

Wasting no time, Aoshi began the operation.

He picked up a scalpel and carefully cut away the bloody bandages, revealing the gaping wound beneath. Fresh blood gushed out, staining her once fair skin.

Thanks to his participation in many operations during his internship, Aoshi immediately determined the cause: a kunai stab.

A nurse quickly stepped forward to clean the area around the wound.

At the same time, Aoshi placed his hands over the wound. Without forming seals, he gathered chakra in his palms, emitting a soft, pale light.

Blood Coagulation Jutsu. A C-Rank medical jutsu, effective for quickly stopping bleeding.

Thanks to his system-enhanced talent, even though he'd only learned it recently, his mastery had already surpassed his basic ninjutsu and reached the proficient level.

He maintained a steady chakra output, and five minutes later, the bleeding finally stopped.

But Aoshi didn't pause. "Apply the medicine."

The nurse moved swiftly, applying the ointment to Mikoto's wound with practiced precision.

Meanwhile, Aoshi activated Healing Jutsu to accelerate the healing process. A D-Rank medical jutsu, this was one of the foundational techniques every medical ninja had to master.

If he had access to the A-Rank Mystic Palm Technique, he wouldn't even need the medicine. With it, he could fully heal the wound in five minutes.

In fact, Healing Jutsu was a simplified version of Mystic Palm, created to popularize medical ninjutsu. The latter was notoriously difficult to learn—even Tsunade, the greatest medical ninja in history, admitted it was no easy feat.

Of course, with Aoshi's system-boosted talent, it was impossible for him not to learn Mystic Palm Jutsu in the future.

But for now, he didn't have access to A-Rank techniques. In Konoha, jutsu ranked B and above were tightly controlled.

There were three main ways to learn them:

Clan Arts – If you were part of a ninja clan, you could learn their inherited jutsu. Apprenticeship – Naruto, for instance, only learned the Rasengan after becoming Jiraiya's student. System Rewards – Aoshi earned the B-Rank Delicate Illness Extraction Technique as a reward for displaying exceptional medical talent.

More than ten minutes passed. By the time Aoshi exhausted his chakra, Mikoto's wound had nearly fully healed. The treatment was complete.

Just then, the system's voice rang out in Aoshi's mind:
